WAIVER, RELEASE OF LIABILITY and PAYMENT AGREEMENT

*  I assume all responsibility for exercising in his or her physical condition.

*  I agree that participation in this program is at my own risk.

*  I agree to waive/release any/all claims arising from services provided.

*  I agree to waive/release any/all claims arising from use of facility/equipment.

*  I accept responsibility for any/all fees attached to the services provided.

*  I approve to be photographed for instructional and promotional purposes.

*  I agree to provide payment in full for all services.

*  I understand there are no refunds/credits for missed sessions.
